In blood, VWF propeptide (VWF:PP) have very different half-lives (12 hours versus 2 hours).
Measurement of VWF: PP is an important tool alongside VWF:AG in characterizing the type of VWF deficiency, especially in patients with shortened plasma half-life of VWF.
VWF is synthesized exclusively by endothelial cells and megakaryocytes in the form of pre-pro-VWF, the propetid of which is eliminated by an enzyme, furin, to give a mature VWF.
The mature VWF will be preserved until its release in the Weibel-Palade bodies in the endothelial cells, while the VWF:PP will be secreted in the plasma. VWF:PP is needed for VWF multimerization and storage.
A mutation in VWF:PP results in a lack of high molecular weight VWF.
